Let's start with his fight with Renji during the time he and Byakuya is going to take Rukia back to SS.
These is Ichigo who was about to fall,during his fight with Renji.What happened next?
Rukia gets worried and she tried to stop Renji and asked Ichigo to leave and run away,what happened next?
Ichigo didn't listen to her....and look at him here....
Ichigo's expression changed....and also these....
Renji was surprised when his reiatsu suddenly rise,though he was almost defeated earlier.....
A second ago Ichigo was almost dying,then he suddenly stands up like he wasn't injured at all,even stronger than before.And seems like he himself didn't notice it....look here.
What is Renji's reaction to these?
Ichigo thought that Renji got slower,he didn't notice that he is the one who suddenly gets faster.
At the SS
Ichigo was about to be killed by Renji once again,however,his spiritual pressure suddenly rose up again (like what happened during their arrest with Rukia),and becomes able to defeat Renji.
Another time that things like these happened again was in these fight....
That is what happened during his fight with Kenpachi at the SS....and it happened once again......what I mean is these......
Once again at the time that Ichigo was about to die,then,he suddenly stands up like nothing happen to him,and his reiatsu was increasing in a tremendous amount,even Kenpachi was surprised to see these....what is the reason of his rising?.....it was because of these...
Ichigo is being persistent of staying alive,trying to continue moving,to stand up and fight,why?.....because of these.
Because of his strong determination to save Rukia,Zangetsu suddenly materialized.....look here.
Did you know how many years it took for a shinigami to be able to materialize their zanpakuto?Let's call on Yoruichi san to tell us about it.
Now,after looking at those fights,did you noticed something?What are those fights have in common?It's Ichigo who was about to be defeated by the enemy,but somehow,his reiatsu suddenly rose up in unfathomable amount that absolutely surprised his enemy.How did it happen?One moment he was about to fall,then the next moment he was standing up once again (with an even stronger reiatsu),like he wasn't injured at all.Did you see him having a power up there?Did he have a special training before continuing the fight?There's none,right?Then,how come he became stronger within a second (just a second)?What is the thing that made him stronger?

I'll try to elaborate things much better....first may I remind you what did Urahara told Ichigo about fighting?
Urahara san,can you tell us once again,what is the important thing that Ichigo should remember every time he was fighting?
So,those are the things that he shouldn't do?......then what is it that he needs to think while he was fighting?
Oh,so those are the important things that he should keep in mind?
Somehow isn't these the same with what Zangetsu told Ichigo here?
Zangetsu said that he should forget about fear,look forward,walk forth,never ever stop,cowardice will cause death....both of them are simply telling him not to be scared,don't hesitate,always move forward,if you fear,you will die.....
in short the essence of their words is simply these....

I see....so that explains what happened during his fight here....
Ichigo was confident here at the beginning,he thought that if he finds out the limit of Renji's attacks,he would definitely win,but he didn't expect that Renji would be able to dodge his attack,because he didn't expect that to happen,he was completely stunned that he didn't manage to dodge Renji's counter attack....And what did Renji told him that time?
Renji thought that he is the one who was going to win,because he was an experienced warrior compared to Ichigo,that made them completely at a different level.Urahara once said these before,that Ichigo was indeed a very strong man,and the only thing that makes the shinigamis having the advantage was their battle experience.But what happened when Ichigo remembered what Urahara have told him?
He won their fight after putting all his will to defeat Renji in his final attack.Did he do any power ups here during his fight with Renji?No he didn't.Then how come he manage to defeat him,though he was having a hard time fighting with him at the beginning?That's it,his rising DETERMINATION gave him enough strength,to defeat Renji.
Let's take a look at what happened in his fight with Kenpachi....
Before their fight have started,Ichigo have already felt Kenpachi's strong reiatsu,it already made him nervous at the beginning,and when he tried to attack him,he was so surprised that he didn't even manage to give him a slightest scratch,even though he used all his strength in his first attempt.What did he do next?
He started to panic,Ichigo got scared of him,then he remembered what Zangetsu told him....
He began to try to calm himself....then he felt Chad's reiatsu fading,what happened then?
Ichigo's DETERMINATION rose up,after remembering the people who are fighting behind his back,'s when he realized that he shouldn't waste his time being scared,because he is not the only one who was fighting,he have regained his courage and faced Kenpachi again....
His determination gave him the strength and manage to cut through Kenpachi,but it wasn't enough to defeat him,that's why these happened....
Ichigo was about to die at that moment,but what gave him strength to stand up and continue to fight?
That's it! his DETERMINATION to save Rukia is the thing that gave him strength and made him won his fight against Kenpachi.Again,does he have any power ups here?There's none,even Zangetsu asked him "Do you want to live,or do you want to win?" he was simply asking about Ichigo's determination.
Now let's see what happened during his fight with Byakuya...
Byakuya was about to kill Ichigo here...what happened next?
His hallow takes over the fight for him,why?Because of these....
He took over because Ichigo was being scared when he witnessed Byakuya's bankai,and it will only lead him to death,and his hallow won't allow that to happen,because if Ichigo die,he will die as well....But Ichigo wants to defeat Byakuya by himself.His strong will to win these battle made him able to do this...
Ichigo regain his control and continue his fight with Byakuya....why was he so determined to won against Byakuya in his own way?
Because of this...
What is that change that he was talking about?
That is what Ichigo wants,he wanted to crash Byakuya's pride,and make him realize that he should protect Rukia no matter what.
According to Urahara and Zangetsu,Ichigo should not feel fear when he was fighting.I guess I will repost Urahara's words here too,to be able to understand what I mean.
Here it is
Like Urahara told him,when he dodge,don't be afraid of being hit,instead,think that he won't be hit.When he was protecting someone,don't be afraid that they might die,think that he won't let them die.When he attacks an enemy,don't hesitate,don't be afraid of cutting them,instead,think that he will definitely cut them.
That is why Ichigo was capable of shoving the hollow off when he tried to interfere during his fight with Byakuya,Ichigo was about to die back then,that's why the hallow appeared,but Ichigo's strong determination to win in his own way,to be able to change something for Rukia's fate,he regained his control,and won against Byakuya.
